Welcome to traveling soccer

In order to participate in traveling soccer, each team must complete and
return an official packet, supplied by the league (not PLRSA). The packets
are usually handed out by your league contact. Once completed, they
must be submitted to the District Commisioner for approval. This is usually
done by the coach. There is also a mandatory coaches meeting prior
to the start of the season (league not PLRSA). At this meeting you will
receive your Game Packet including you came cards.

All coaches must be licensed and have at least a "F" license.

Your Traveling Soccer Registration Packet should include

Team:
1 Roster* (comes with official packet)

Players:
1 Medical Release Form (Notarized) available on-line
here
1 Application/Membership Form (signed by player and parent) available on-line here
1 Picture 1"x1" (to be affixed to player pass)
1 Copy of the players birth certificate as proof of age
1 Player Pass (signed by player)*
1 SAGE Form (signed by player/parent/coaches)

Coaches:
1 Membership Form (same as Application) available on-line

1 Coaches Pass 1 Picture (affixed to pass)*
1 Copy of "F" license
1 SAGE Form

* These items will not be available until the official packet arrives
In the meantime you can get started on the rest

NOTE:
No White-out or corrections are allowed you must do this right the first time
if a player or parent made corrections, you need to redo the form

The packet must then be submitted to the district commisioner for approval
and stamping.

Secondary cards! Extremely Difficult Process-

- PLRSA does not permit secondary Cards
Secondary cards can only be obtained from New Jersey Youth Soccer (not from MCYSA).
Send a note of the request, along with a $10 check (per card) to:
New Jersey Youth Soccer

Hightstown, NJ 08520

1. Once you receive the card- also obtain ad Add/Release form
2. Fill out the form and the Card
3. Have the coach from the opposing team sign his/her section
4. Sign your section
5. Obtain a copy of the players primary card (front and back)
6. Send Form and card, along with copy of primary card to the MCYSA Registrar for signature
7. Send everything to NJYS for signature and processing
8. Send everything to the local DC (District Commissioner) for signature and stamping.
You are done!

Remember - the players primary obligation is to his/her primary team. Also, State Cup and many official tournaments will not accept secondary carded players.
